240VAC on the ground or grounded conductor (which ever the center light is for)?

I have had all lights go bright on me. Tested out as running 240VAC.

I don't know the type of tester your using...

How old is the house?  Based on the test button working, it is most likely a home that had early 2 wire system without ground.  If the GFCI was not wired properly, you would not have been able to reset it.

The proper test is with the test button on the GFCI.  A 3 light tester requires a ground to function.

There is nothing sweeter than the smell of fresh cut grass on a baseball infield, the click of a wooden bat and the taste of a hot dog at a warm sunny daytime double-header.

As long as it tested good with the button, that's all that matters.  That is to say, the manufacturer's designed test method is the one that counts.
